Fees and Insurance

Medicare

We accept fee assignments from Medicare. Claims will be submitted directly to Medicare by our office. If you have secondary insurance coverage, a claim will be submitted after the Medicare payment is received.

Other Insurance

"The insurance plans we participate with are:  Aetna, Blue Cross Blue Shield Traditional and PPO, BCN, BCN Advantage, Cofinity, HAP, Priority Health, Medicare and some  Medicare Advantage Plans.  Patients with these policies are responsible for copays and deductibles.  For any other insurance coverage, payment is expected at the time of service.  As a courtesy, we will file claims to your insurance company for your reimbursement.

HMO’S

Your HMO requires that your Primary Care Physician (PCP) authorize all visits to a specialist before such visits occur. It is your responsibility to obtain an authorization from your PCP for each visit. If you do not have an authorization from your PCP at the time of service, you may be asked to reschedule or to sign a waiver agreeing to assume all financial responsibility for charges associated with the visit, including all charges for treatment, x-rays, casting, injections, etc.

Our Fees

We request payment for our services at the time of your visit. If you have additional questions about payment or payment arrangement, our billing office is open everyday and happy to discuss any concerns with you.
